PAYMENT POLICY:
1. PAYMENT IN FULL IS DUE WHEN SERVICES ARE RENDERED (i.e. Completion of the appointment and/or discharge of the patient from the hospital) OR PRODUCTS ARE PURCHASED.
2. Payments are accepted in the form of cash, check, valid credit card or CareCredit.
3. DCLAH does not offer payment plans.
4. A service fee of $20 will be implemented on all accounts over 30 days old. Any account that exceeds 60 days of non-payment will no longer be offered services until the account is made whole.
5. All accounts over 120 days non-payment will be sent to collections and clinicians notify
.

Payment is due at time of services rendered. We accept cash, in-state personal checks and all major credit cards. This includes MasterCard, Visa, Discover, American Express, and CareCredit.
*A 3% processing fee will apply to all credit card transactions.
